I, full legal name below, fully understand and agree to assume all risks involved in any and all duties that I perform as a Volunteer in my volunteer capacity and I agree to hold No Wagging Tail Left Behind Dog Rescue ("NWTLB") harmless for any injury /injuries which I might sustain during the course of my volunteer duties. This waiver does include myself, all members of my family, family pets, descendants and property forever from seeking any legal action whatsoever against NWTLB or its representatives.*

I understand that I may not disclose or use proprietary or confidential information of NWTLB except in the course as my direct volunteer activities requires. *

Proprietary Information includes all information obtained by Volunteers during the course of their Volunteer activities. Confidential information is any information that is not generally known to the public. Business and marketing plans, family and volunteer information, literature, veterinary list, training systems, contact information, policies and procedures, vendor list, computer records, financial data, internet transmission, other electronic communications, passwords to any and all electronic formats are examples of confidential information and included in this Agreement.*

I hereby release, authorize and give full consent to NWTLB to publish and display my photograph in which myself, or any member of my family, including pets, may appear. It is further agreed that NWTLB may use or cause to be used such material for, or in, visual displays, any exhibits, internet web pages, social media outlets, publications, or other marketing collateral. I also acknowledge that I have received no monetary compensation for material used pursuant to this release nor do I intend to seek any such compensation. I also declare that by my typed full legal name included in this Agreement, that this testimony is factual and accurate.*
